Veterans and First Responders need easy access to specialized services, programs and the latest technologies without barriers. The Centre of Clinical Excellence has brought together expert practitioners and clinics to make meaningful, lasting change in the health & wellness of this deserving population. We will provide multidisciplinary, best-in-class healthcare by integrating a continuum of health services for Veterans, First Responders, and their spouses. Services will be in our state-of-the-art facility in the Legion Veterans Village in Surrey, BC, and through virtual care. Continuity of care — Access to primary care ensures that Veterans and First Responders receive consistent and ongoing medical attention. This continuity is essential for managing chronic conditions and preventing potential health issues. Preventive care — Primary care providers can help Veterans and First Responders stay healthy by conducting regular check-ups, screenings, and vaccinations. This prevents diseases and complications before they occur. Management of physical & mental health conditions — Primary care providers play a crucial role in managing both physical and mental health conditions commonly experienced by Veterans and First Responders, such as PTSD, traumatic brain injuries, and musculoskeletal problems. Coordination of care — Primary care providers serve as the central point of contact for coordinating care between specialists and healthcare providers. This ensures that Veterans' and First Responders' health needs are met holistically, comprehensively and in a timely manner. Health education and support — Primary care providers educate about healthy lifestyle choices, provide counselling on mental health issues, and offer support for managing chronic conditions.
